My friend has been using my horse once weekly for her lessons and has noticed a few times that he will kick out with one of his back legs and then doesnt do it again the entire time he's ridden. Could this be a hock issue? He was previously injected in the left hock but the right one was xrayed and fine. I haven't ridden in almost two months due to my breast reduction so I have no idea what his issue is, but I'm thinking hocks

It sounds more like a chiropractic issue to me. You say it happens after he is mounted, which to me sounds like something hurts as he is being mounted and then it releases afterwards and so he kicks out.

His back always seems to be sore when he gets adjusted. So may they are hitting a sore spot and the kicking releases the pain? That makes sense I just want to treat him accordingly

My mare would kick out with one back leg when I asked her to run, turns out she has kissing spine. It was diagnosed while she was turned out for a stifle injury so I haven't run her yet to see if she will still kick out.
I always thought she was bucking just a little until we caught it on camera. She very distinctly will fire with her left rear, like a cow kick when she hits a certain speed. her saddle also had too much rock for her back and it put pressure right over the spot we injected. The sore spot was very visible with a thermography camera, if you can find a vet that has one it would be the cheapest way to figure out if it is a back issue or something else.

How big is your friend in the height and weight departments .... ???
May be as simple as girth pinch if rider has to hang on to saddle, twist saddle, or does not mount with a smooth motion .... or horse gets poked with her toe ... type of girth being used vs yours ... or even if friend uses reins for balance and to pull herself into the saddle and jerks horses mouth with one or both reins ...
Think simple things ... the reaction is right after an irritating moment to the horse ... Girth too tight?? ... saddled low behind withers ... etc etc ...
