 Why is it some people absolutely won't go to the doctor? 

Person I know has very, very long and very hard and yellowed toe nails.  The big toe nail is so long and pointed that it goes through socks.  It has to be uncomfortable, but they will not go to the doctor to get it removed.  We've tried everything to cut it, but it's too hard and thick.  I'm pretty sure the foot doctor would have some sort of cutting tool, because I'm sure this isn't rare. 

Person does go to doctor for other things, but won't mention this to them.  Any ideas for cutting it?

Maybe they are embarrassed?

I almost refuse to go to the doctor anymore because over half the time I leave there without any answers and still have problems.... plus a big bill.

Dremmel oughta do the trick.

I think embarrassment mostly. Or fear of what they will find and be told. Denial over their current lifestyle and not wanting to hear that they need to make changes or risk further health decline.
