Has anyone ever seen anything like this? It has supposedly been this way for years. There is an actual open hole.

Yep.  Got one myself.  Smaller hole though.  With horses, sometimes letting things alone is better then the alternative.  We could easily do a bone graft surgery on my guy, but the risk of complete sedation and surgery are not worth it.  Having a small hole in his face right now isn't hurting anything.

Had a dog get rattlesnake bit on the side of his mouth. Never healed. He did just fine though. It left a 2 inch hole so he was a messy eater and drinker, but he kept his weight just fine. This dog was terrified of traveling so I had the vet come out. He said he would heal, just would always have the hole but since he was so terrified of travel, the vet recommended leaving it.
