I would use oroquin from dr. Ellerison, from fla. Her treatment is like 200.00 sure helped my horse. He was chronically sore did this no soreness at all.

My vet uses Oroquin. He says that's not necessarily a relapse, but an.... inflammatory response? and there is another treatment you give if it happens. I think it costs around $100 or so. Not every horse needs it.
He also said he has about quit using anything else because the number of horses that relapse later on is lower with the Oroquin than other treatments.

I know every horse is different and EPM is tough to treat. I do know I treated with oroquin and several months later my horse had atrophy of her masseter muscle. We retreated with marquis and she regenerated the nerve that supplied her masseter and regained her muscle tone. I know not every case works that way.
I won't rule out any treatments... diclazural and toltrazuril are also used, although I don't see a point in using toltrazuril since it metabolizes to ponazuril which is what marquis is.
Nitazoxanide is also labeled for EPM but I don't know what commercial product that is or if its in the US.
whats the active ingredient for oroquin?
We had another horse treated with some weird concoction a vet made up with lord knows what in it, and it worked.. LOL
