Before I give any more info. Does he look lame to you?
https://www.youtube.com/watch?v=2z_SMFaQkIo

Vet was out on March 5th as he was very lame. Did a flexion test, nothing bothered him, no swelling/sensitivity/ heat in the leg. Tried to block him out just above the hoof but he reaced too badly to the needle so they couldn't. X-rayed his hoof and nothing was amiss. He was sensitive on his right hind toe when they used the hoof testers. Farrier trimmed him yesterday, but I wasn't out there so I didn't get the chance to ask if he could check if he was sensitive. I did soak his hoof for a week after the vet was out (per her instructions) but weren't able to get an abscess to come to the surface.  I'm going to send this to my vet in the morning. Just thought I'd get some more opinions beforehand.

There is something, perhaps stifle, as when he went to stop he stuck his toe into the ground, I was told this is a stifle issue.

Ok he still looks off to me too but my farrier didn't see it. I've delt with so much lameness I can't trust my own eyes anymore. Whatever it is can't be hurting him THAT badly. After I took this video my sister let him roll in the round pen and he took off running and bucking for a few minutes straight.

Ahhhhhh now that would explain why he didn't look any different after the flexion test. Right before we really noticed the lameness he was having a hard time picking up his right lead and cross cantering a lot.

We decided to wait on the x-rays and see if he just had an abscess (going back to the tender toe). Once I arrange for someone with a trailer to give us a ride I'll be taking him back there to further investigate what's troubling him.

In these situations ... squinting your eyes really does help!!

When I squint my 3rd eye .. I see a suspensory problem in the RR ankle to foot support ...

From the start of the video ...>>
At the walk his RR toe/hoof dangles when he takes a step so he short steps on the RR in order for hoof to land flat ...
Watch his walking turn very carefully .. as he moves his rear his RR steps outside his frame with a limp and then his RR bears some weight and you can see it give way and his LR moves into position to protect the RR movement.

Left side looks clean and then the turnaround from a trot you see the horse do a flinch stumble when he uses the RR to make the stop. As he comes by the camera you can plainly see him short stepping and landing on his toe with his RR ..

The third time was disorganized so no comment on it ...

Do not be deceived by him using his LR to support the problem in his RR and to favor his bad foot on the RR.

GOOD LUCK .. 3-6 months time on pasture rest is your best friend !!

The tender toe was on his right hind. But I couldn't tell which leg it was by watching! The vet did check both legs but he didn't react at all to flexion tests in either leg (and he is a VERY reactive horse) so without actually being able to block him out (he was twitched and sedated and still running around the arena with a needle sticking out of his leg ) or do x-rays and/or ultrasound randomly we don't know absolutely for sure which leg it was.

The kicker is, he has been doing NOTHING. He's kept in a run by himself during the day and a stall at night. He was only getting ridden lightly a few times a week by my sister.

Have you had him chiro or massaged? My gelding is the king at coming out of his stall lame. Nothing you can really see just what you can feel. Had him chiro every 4 weeks and he got a massage every 2 weeks. Had muscle tenderness in his lower back and left hip. Tightness in his chest and under arm as well as is abdomen. Good luck! Hate mystery cases.

I did have her trot him in one of the outdoors, which is not super deep but still, he looked slightly worse in that.

Right hind.  He's not pulling it underneath himself.  I would have hocks checked and go from there, since that is most common.

I hope it's just a bit of hock arthritis. We were just about to start prepping this horse to sell when he came up lame. At least hocks can be managed.

Xray that stifle..... Just got back from the vet. we were so fixated on our mares scar on her ankle I never thought to check it.... Her stifle is trashed... fractures/bone chips. We know when it happened but not how or why so bad. So she is full on retired now (been semi retired for 3 years and was just rode once just before she got this hurt). NOT that your gelding is that bad! Im just sad lol.

That is awful! I hope that's not the case with my gelding. We can't afford to keep a useless horse around. We board.
