I tested my gelding almost a year ago to the date, through pathogens because I suspected EPM. He came back with SAG1 2 SAG5 32 SAG6 8
I treated him with Origin for 10 days as recommended by Dr. Ellison.
Noticed very slight improvement if any, and kinda chalked it off as him just being lazy. He is dipping into the 1-D but not consistent. Seems if he doesn't catch his hind lead going to the second barrel he panics and bows out. But if he does catch it, he is top of the 1-d. He has only been entered about 15 times.  
Took him to a barrel race last weekend and he was totally off. Very agitated, nervous and not his laid back self. Took him in and my vet recommended sending a sample to UC Davis for another opinion and start him on Marquis.
Got results back yesterday, and he was NEGATIVE!!!!!!!
Havent talked to the vet yet, but wondering what you all would do? I AM SOOO CONFUSED! Please help

Maybe he is sore or just slightly out..I'd try some chiro and/or acupuncture...i know sounds like voodoo but the way it sounds like maybe he's sore or out somewhere and when you leave the 2nd is maybe when it's really pinching him...looks like your titers before epm treatment were high and he likely had it then the orogin did it's job and got rid of it but maybe from traveling off with the epm he got out ...also orogin has Levamisole in it which does have anti inflammatory properties so it likely helped at first with whatever inflammation he had but now you've re-agitated the area that's out and it needs some work and maybe anti inflammatory to help.. ...also you might check with dr.ellison but sometimes they recommend Levamisole being given for a little while after to help with residual inflammation.. epm protozoa tend to cause a lot of inflammation along the spine because that's where they hang out in the spinal fluid...even after the boogers are dead it takes a while for the inflammation to reside partly because of the damage and irritation they do and also because while the are dead their little bodies don't just go away they are slowly absorbed by the horses body but until they are they are floating in the spinal fluid still causing inflammation until they are absorbed and also increased spinal fluid pressure ...spinal fluid is in a closed system so it takes a while..your epm test came back negative because they tested the blood and there aren't any there or enough antibodies to indicate an active infection however if you pulled spinal fluid there likely would be residual dead protozoa there..Dr Ellison can/will RX more Levamisole to help reduce the inflammation..good news is Levamisole is safe and they even use it in human cancer patients to reduce inflammation and stimulate the immune system and it's not overly expensive.

 Would you test him through pathogens again? I haven't had him adjusted since last year, so maybe that would help. He's not sore anywhere that my vet noticed. So frustrating!

I edited and added some more but yea I'd call to see if they recommend treatment with just Levamisole..see above :) orogin is Levamisole and decoquinate combo but since the infection is dead likely still needs Levamisole for a bit longer..

Don't believe the test. Mine was the same thing. After a general exam with the usual tests for EPM, tail pull, lean back hoof, etc the vet described his condition as 'classic epm', we started treatement immediately, the test came back negative. Treated for 3 rounds of compound drug, and he's back to his old self. Good luck

As I understand it the false positives are about 20 to 30 percent and false negatives are about 10 to 15 percent, so the test is about 50/50....I don't think I'll ever test another one, I'll just treat.

 I would contact Dr. Ellison and see what she thinks.  The Neuroquel follow up treatment that she does is Levamisole.  She may have a totally different recommendation though.  I wouldn't just be throwing random drugs at the problem since you just treated with Orogin.

 Treat him again, either with pathogenes product or another... EPM treatment is cheap, and epm-ish symptoms are so prevelant. I treat most of mine, as do most people with a clue... I wouldnt spend my money on marquis, too pricey... Is he back sore?

Levamisole really seemed to help my mare after the Origin treatment. She was doing good, but really seemed to bounce back after the Levamisole.
