Your time and complete attention for a few hours.  
A digital photo frames that you can load 100's of pictures on.
Bring him a nice dinner and sit and chat.
Sit with him and write down his adventures from his youth, kind of a living album.  The gift is more for you, but he might really enjoy sharing his memories.

 My Mom is right there. I every year get her good slippers from Cabellas, and new pajamas and a gift card for dinner out. Then other odd and end goodies like a good blanket while she sits in her chair watching tv. Stuff to keep her warm and comfortable and whatever to make her life more enjoyable and comfortable being house bound. Oh and last year we also got her a tray on wheels ( kinda like the old timey tv trays )  but nice made of solid wood so she can eat while in her chair.

Absolutely agree with the time thing, but that's out. (Edited to add, meant to quote you AM) He's 24 hours away, and a trip is out of the question. I wouldn't buy him anything if I was going to see him, BUT...here we are.

I did score him a really cool gift basket with everything under the sun to eat, hopefully that goes over well.

Also got a really nice throw blanket, very snuggly.

Would it be tacky to send a framed picture of myself?

I like what others have said! If they read, maybe those lights that clip on to what you're reading. Maybe a gag gift to make them laugh, an ornament, a bible, a nice blanket for their lap and you could make that too!

If he likes old movies, perhaps you could buy several for him. My grandfather loved old Westerns -- and when he became house-bound, we supplied him with all we could find - he loved John Wayne and we bought eveyr one of the John Wayne westerns for him. He also liked Gunsmoke and we bought tapes of it, too. The best gift of all could be several movies and you making time to watch them with him along with providing some of his favorite snacks!

  as for the picture of yourself. My grandpa is not house bound and I can't send him pictures on his phone. So I bought him a photo album for his birthday, about the size of a woman's wallet. I filled it with pictures of my little family, mainly the kids though. He made a joke about it that for my birthday I was getting a photo album full of pictures of him lol. He jokes but he keeps it in his truck so he can show off his grandkids to all his buddies that he meets up with every day. On Thanksgiving he even busted it out and had all of us look through it again lol. Last year I made him a quilt. He went and bought a chair to lay it on that would match it perfectly. That chair is like the centerpiece for his living room lol.My mom is his only child, he is no longer married, me and my sister are only grandkids and my 2 are only great grandkids. He really enjoys showing us off. Even takes us to lunch at his hang out so he can tell everyone who we are lol. Family is big to him.
