I am having a very hard time with my guy... He is 16hh with a fairly high long wither but a flat back. I have tried double js, Martins, Semas saddles, cactus, and a few others but everything I try either has too much rock in the tree or doesn't have enough clearance on the wither! Any suggestions would be greatly appreciated, I'm even willing to get a custom fitted saddle built at this point if anybody has recommendations for really good quality custom saddles.

Maybe try a Robbie Phillips, he had a booth set up at last years BBR. He offers a variety of tree options and will help you fit your horse.

Did you try a Martin flat tree, these are custom order ones, but they did have a few in the loaner program

I tried a crown c with a flat tree but didn't have enough clearance at the wither. When we put the shims on to level out the saddle I couldn't even get one finger under there because of his long wither.

Does the horse have muscle atrophy by the wither?

I was going to suggest a Shiloh or the Robbie Phillips.

No he doesn't. I will see if I can get some photos today. I haven't tried a Caldwell yet so was thinking maybe they might have something that would work?
