Has anybody see this before? It is an older horse doesn't get rode so it isnt a saddle and he has had it at least a year that I have known him. It is the only spot on him and it is behind the wither. It is greasy feeling and he doesn't like you messing with it.

Could be a sarcoid.
Types of sarcoid
Occult: a flat patch of hair loss with a grey, scaly surface, which can be confused with ringworm, as they are often circular. Common on the face, neck and between the back legs

I was raised around hard working draft mules and horses snaking downed trees to portable sawmills which really asked for their heart and all the muscle stamina they had ....
We had a few that would have FISTULA which is a complicated internal infection or strain that eventually comes to the surface at the withers ... as a greasy type sore from the internal infection ..... in the 50's and 60's we did not have the antibiotics that are available today ....
COMBIOTIC was our most successful shortest term antibiotic ... it had both positive and negative drugs to attack about anything a horse could have ......... and of course it worked so well ... it was taken off the market ... discuss the +/- with your vet .... I haven't found anything similar to the effectiveness of the old drug named COMBIOTIC that attacks the +/- aspects available ... ...

I agree it looks like a sarcoid. My mare had one on her neck for a couple years, it never changed or spread but I was going to have it removed this past fall atter the bugs went away but then last fall when her winter coat came in the sarcoid went away. Happy I didn't have to spend the money to get it removed but I didn't think they went away just like that. Ohhh well
