When I was working for the vets during the big outbreak around here they had good success if the horse stayed on its feet.  It seemed like if they got down they didn't get back up.  Yes they were treated with lots of DMSO.  The whole clinic would smell of garlic when we had one of those horses staying.

I have a 3 yr old son of Frenchmans Guy, currently in barrel training in Oregon. He had been vaccinated with all the typical vaccines last year but I had mistakenly decided WNV was not an issue so he was not vaccinated. I got a call 3 weeks into his training last August that he was not right - not tracking with his back legs - the trainer watched him close for a couple hours and then hauled him to the vet that afternoon. Trainer called from the vet's ofc later, asked for me to confirm the horse had not received any WNV vaccination, and told me they suspected he had it. Blood work confirmed it positive 10 days later. They started him on Banamine - 5 cc 2x a day - not a pain mgmt dose but as an anti-inflammatory. I did hear DMSO drips were an option but he didnt seem to get worse so after 7-10 days I brought him home. I was surprised how lithargic he was. He drug his hind feet instead of picking them up when he walked. Otherwise he seemed just weak ... not a strong appetite. i bought Natural Flax and we added Platinim vitamin E and just let him rest. He was not himself for a couple weeks - I prayed and knew it would be 6 months before I knew if he would be the 20% with no issues, or 30% with residual effect or 50% that died or had to be put down. I think it took4-5 months before there were no symptoms and he is totally well and back in training. I did give him a West Nile vaccination this year (just 1) as vets are suggesting that. The trainer was 5 hrs from me and when I went to pick him up last summer, the locals told me more than 30 horses had died in that area from WNV. I pray your friend's horse gets well!!

Back in about 2002-2003 we had a broodmare get it when it was big in our area.  We were told the same thing that if she stayed on her feet we would be okay but if she went down she was probably a goner.  I know they did DMSO IV on her.  She stayed at the vet's for about a week and we were then able to bring her home.  She lost a ton of weight and was still dragging a hind leg when brought home.  But I don't remember seeing any lingering side affects after that first few months with her.  She is 32 this year.

Several years ago we had a mare have it and live. She fully recovered from it. We treated her with DMSO & Banamine. I also put Flax  seed in her feed.
You have got to keep their fever down. And early treatment it the key to recovery one day can make all the difference. If she even thinks her horse has it she already needs to be treating it
A few years before that we had a gelding get it and we had to put him down. Both horses had been vaccinated for it

I rode a filly for a man whose stud horse ended up positive for WNV. After like several months of treatment, he made a full recovery with no issues. He uses him in the sale barn, along with roping off him.

It can be treated if caught early, but not all horses can pull out of it. Had one on my place that had it when he was bought didnt show any signs of it untill a few days later and then didnt relize he had West Niles untill his blood work came back and then it was too late so had to put him down, this was like 25 years ago.. If I ever had another one that I thought it had west niles I would start treatment as soon as possible..

I've heard of a lot of them recovering - some 100% fine as far as everyone can tell. But, there are a few that have residual mental/anxiety/neural issues afterwards. I've had discussions with several people about it making their horses "stupid" after they recover.

I had one in '02 or '03 that got it. Caught it quick he would just drag a leg every few steps in the hind end when we took him in. Vet treated with 10cc of Banamine twice a day for 10 days. He came out of it ok.
