horse.com has some cheaper built up cutback pads to start with.
If you know exactly what you need, team equine will build you a custom pad for your horse and they are great to work with.
there are alot of pads out there. I think reinsman has a built up pad with a "pocket" for adding shims. Diamond wool has a pad with shims (3 on each side). impact gel has a built up pad if that's what you like.... there are alot....

Classic Equine has a build up pad for high withered horses.. It's what I use. Works great on mine

My old horse was extremely high withered, built like a thoroughbred or warm blood almost, and was also very narrow too. I used a Molly Powell X series saddle and a Tod Slone cutout pad I think is 1" or 1 1/8" and that fit him perfectly! The saddle was raised high enough in the front it never got close to his withers. And I loved riding in that saddle as well.

Shiloh saddle (Contender, Ed Wright or Jerri Mann) and a Saddleright pad. If I could get a deal on a TG Caldwell I would also go that route. I had a Lynn McKenzie Special and it SAT on my horses withers, did not have a high enough gullet.

I use a Diamond Wool Pressure Relief pad for my gelding with high withers. It was well worth the price tag.

 a high withered horse does not usually need extra padding or a built up saddle pad unless they are sunken in or sway backed and you need to level the saddle out. Find a pad with cut out withers or reinsman has a pad where the entire front isn't connected and the pad only covers the shoulders and back. Make sure your saddle fits without a pad and your pad should only have to be a 1/2" thick.
