My horse started to tilt at the poll. We got her teeth looked out and they got floated. We thought that would take care of it but it didnt. She is still doing it. She did it at a walk and trot without putting any pressure on the but. We decided to lunge her under saddle with just a halter to see if it was still her mouth. She did it as much bit less as she did with the bit. What could be causing the tilting?

My mare carried her head just slightly to the right I could straighten her up but she would revert back this went on for a while no biggie she was running well. eventually we found out why but it never got my attention untill she quit doing it.
She had broken her left hind splint bone the problem were the calcium deposits that had formed a huge knott that took 6 months to a year to develop. Her peformance was still ok at times but she goofed up her second run in the same day every time. They told me the calcalfication was embeded into the suspensory ligement
DVM found it and Mississippi State U did the surgery to remove it, she was on stall rest for 4 weeks with a daily hand walking schudle. She is fine now but looking back the first time I rode her I noticed she kept her head straight and that was the moment I realized she was telling me for a long time something was off

how long? my horse was doing it - had vet check teeth, hocks..all good. we did chiro in case he was out at poll and it seemed to help a little, but then started again. Went on for about three weeks - and now he seems to have an abcess/gravel in right hind.  I am betting that once the abcess is gone it stops this also. am hoping that was what was bugging him all along.

She doesn't do it the whole time. Just a couple times every time I ride or lunge. And it's always to the same side.

My mare did this and we did multiple dental exams and chiro treatments which would help short term. It kept coming back and getting worse. finally figured out she had EPM. She had always been a stumbly horse but I never thought much about it until we put it all together and were like "Aha!"

How is she on placing her feet? Does she stumble?

The head tilting could mean a few things, the bit maybe pinching, her back could be hurt, tmj, she had a blow to her head, are EPM that would be the first to be testing for. One of my geldings just out of the blue was standing with his head tilted and then it got worst he could not stand was falling down, he had ran into a tree during the night when the other horse went to a different spot and he was being a dork looking for them this is what I was thinking but no marks on his head, but wanted to rule out EPM so had him tested no EPM but his brain swelled had him on meds and then he did get better it took about 8 months to over his head injury and the tilt went away finally.
Hes the one in my Avatar, he went back to running barrels after his boo boo.

Poll or TMJ can cause this..did she have it prior to dentist work? If horse even tweaked her poll while having dentist work done it could have done this..... Id have chiro adjust her . does horse also shake head a lil when bitted up.
if she does have TMJ then dentist needs to readjust the Teeth alignment and space...under sedation and carefully so she doesnt pull for it will make it inflamed more .. it will worsen it...but can take up to 4 months of chiro and accupuncture and advise to not work her..maybe lightly no bit..

My mare did this for a while (when I rode and lunged her)...I have her teeth done regularly, so I called the chiro...nothing out of the norm. Turns out she wasnt balanced in her shoulders. Lunging in side reins to help her from dropping her shoulder and supporting it, and doing the same while riding her (so. many. large. circles.)...She isnt perfect yet, but is better.

She doesn't stumble. She does it without the bit too. She is a five year old.

Can you post video? TMJ they dont have to have a bit in mouth ... shoulder issue as someone said or weakness can also make them tilt or be downhill on one side.. but hard to say.. could be several reasons

Why dont you just go have her tested for EPM? And rule out any soundness issues..That would be a start

I can try to get a video while I'm lunging her. She doesn't do it the whole time. She usually does it a pretty good amount but I just lunged her and she only did it three times.

I have a mare that does it when her hocks need injected. I would say take her to a vet and see if they can find somewhere she is in pain and I would take her to Chiroprator

I looked at the epm signs and she has none and she is stalled all the time with occasional turnout in the outdoor arena.
I am friends with the people that had her and she is still over at their barn. They said shenever did it . She just started it. We found a laceration in her mouth from a point on her tooth and got it correct, thinking that was it.

And I checked her back and her back is not out. And she is setting up and not trying to favor any of her legs.

I do not sure if this information will help. I did not put this in the very first post. She is a five year old racehorse. She had 20 starts. I started riding her while she was still on the track. She officially got off the track a month ago.
