experiences with fetlock infection?

Looking for anyone with experiences with an infection inside the fetlock.
For the last couple years my 19 year old mare has been dealing with cellulitus in her rear leg, but aside from the swollen look and a slight loss of flexibility she was never lame on it. She has been running better than ever, with a much reduced work load, and seemed like the more she was worked the more it helped keep the swelling out of the leg. Four weeks ago we went to the first big race of the year and she was running very good, won 1D money in both poles and barrels in a strong field.
The night after returning her sister in the stall next to her colic'ed and was on her back in the stall kick around. We got the colic'ing horse up and she pooped and was essentially fine. I go back in the remove the ice boots from my mare and she is essentially 3 legged. Not sure how she hurt herself as the boots are very padded and fully covered the fetlock, maybe kicked the wall or something.
The leg with cellulitus is starting to really swell up and she is very lame, not terribly concerned as she goes through increased swelling a couple times a year though usually not lame. Next morning very swollen from hock down and very lame.
Vet comes out xray's everything all good. He is concerned about infection in the joint but does not want to put a needle in the joint because of the swelling and risk of contaminating the joint. Put her on antibiotics, dex and pressure wrap. We keep this up for 3 weeks with the swelling gradually going down, but she still is essentially 3 legged.
So last night swelling was down enough. Ultra sounds did not show much. He got a needle in the joint fluid was very yellow and cloudy. He injected antibiotics and some kind of pain killer into the joint and 10 minutes later she was walking normal and putting weight on it. He took some blood and used a hand held tester that tested for infection in the blood stream, came back 450 where as healthy should be close to zero. The joint fluid is going to the lab and will hear back in a couple days.
It is really looking like the infection is deep in the fetlock. He is talking like the only real treatment is flushing the whole joint several times and hoping to get all of the infection out. Very expensive and the other added complication is the cellulitus in this leg will make that even more challenging as this horse needs to be kept moving every day to pump the fluids out of her leg or it just ballons up.
Any experiences or ideas?
