| Working to rule out where the lameness is on my horse. Symptoms:
- intermittent (sound to dead lame)
- front right (tries to keep off the inside hoof wall)
- no heat in leg anywhere
- hoof testers negative for any ouchy spots
- blocks in the past showed heel
- x-rays are clea?n
I am completely stumped? on where to look

Location: Texas! | My horse was the same, rads showed nothing but a slight unevenness in coffin joint spacing due to him being pigeon toed. Injected coffin joint and some pentaussie and bam! He's back! |

I am completely stumped? on where to look
If the blocks showed heel, it could be contracted heels, navicular, have seen a horse who was on and off lame in foot for a year and it was an infected navicular bursa sac, sorry I can't remember how that one was diagnosed. |

Very similar issue with my horse, except it was his short pastern joint that showed uneven joint spaces. Also is pigeon-toed.
He came up slightly lame on both front feet this year, with the left being worse. 100% sound with a heel block.
So this year we did a 2 degree wedge shoe with pad and he was 100% sound. I'm going to try regular shoes in the spring and see how he does with that. May never need to do injections as long as the shoes work. The shoes enable us to slightly offset them to the outside, to help him land/load more evenly on his pigeon-toed feet.
(But now he's got a catching right stifle issue, which I want to inject in the spring because exercise/conditioning is not working.) |

| here are the x-rays. I see a difference in the navicular from the right to the left...anyone else?

| On the left X-ray on the pastern there is a line, looks like bone remodelling, any fractures.
Also too hard to look at only one angle, my vet always does min 3 views.
I do see your horse has too much toe. Not sure about the black line running down the coffin, this might me the start of laminitis, I can't remember for sure, but I would speak to your vet |

| My horse was slightly "off" on the front. Acted ouchy after he ran, like his feet stung. His toes were long like yours. Cut them back and got the foot back underneath him. He feels much better and not ouchy. Show extras to farrier so he has a guide. Good luck.

I saw a line on the right pastern and to me it looked like a blood vessel. I didn't see the one on the left.
ETA- I see the line on the left too. My guess is still blood vessel since its apparent on both side.s

Location: So. California | I have no suggestions, but wish you luck in finding the lameness.
I, too, have a horse with undetermined lameness and it can be very frustrating to say the least. 18 months apart, my mare came up really lame, 3+/5 lame on the left front, 2 vets, 2 different clinics, 2 sets of x-rays.. and notta... 
1. X-rayed both front feet, clean.
2. No heat, no swelling, no recent physical injury that can be seen.
3. No reaction to hoof tester
4. Ultrasound was clean, tendons and ligaments look great.
5. Sound when blocked at PD
6. Farrier great, both vets said do not change a thing.
She has an old scar on the back of her pastern from when she was 2, she is 8 now. That "may" have something to do with it, may not.  Next step is a standing MRI, working on saving my pennies for that procedure. Sure do wish they could talk...

Location: Benton City, WA | Sounds like you need an MRI of the hoof. There are many soft tissue structures within the hoof capsule that don't show up on an xray, and when injured, can cause the symptoms you describe.
What did your vet say?? |

I agree about needing a MRI of the hoof. I just had a friend go threw this. Couldn't figure out what was wrong for several months. Finally went and had a MRI done.. there was plenty of damage and now they're trying to repair it. All the damage was done down in the hoof so nothing ever showed up on the ultrasounds or x-rays. |

An x-ray will tell you nothing about the soft tissue in the foot. An MRI is the only way to view it and see where damage is being done.
