I believe it's done with a special plate hitch that is attached to the rear frame of the front trailer.

My cousin used to haul trailers for NRS, and this is what they did...
A bumper hitch/ball is installed on the first trailer (like what would be on a pickup to pull a bumper bull) and to that is attached a tongue with an axle and wheels/tires. There is a platform/plate riding on that that has a gooseneck ball just like what's in the bed of a pickup. That's what the second trailer is attached to.
