We treat with sand clear at least once a year. The horses that really pick through the dirt/sand will get treated more often if needed. We have been really dry for several years and even in the irrigated pastures the horses seem to be picking up a lot of sand. This past year I had two younger horses that appeared to be sand colicing. I treated both of them for two weeks just to make sure we got the sand situation taken care of. Prior to this, we've only had one other horse sand colic, and that was around 7 years ago.

The recommended dose for sand clear is once monthly, so do you do the recommended dose once a year, or do you increase?

I just do the recommended dose, one scoop a day for a week. We are very sandy and have had very few problems doing it this way. Our horses hate it, and I'm afraid that if I tried to do it monthly they'd quit eating their grain completely!
If you're concerned about the amount of sand in your horse and how frequently you need to treat, you could just check his manure regularly. Pick a piece from the top of the fresh pile
(one that hasn't made contact with the ground
) and stick it in a baggy or OB glove, fill with water, mush it up, and let it settle. The sand will settle at the bottom.

And do Metamucil

) for 7 days every few months.

Bran mash can get your cal/phos ratio out of whack
Sand clear works,
Beet pulp helps
Whole Flax seed helps

Not only can bran mess up the calcium/phosporous ratio, but bran WILL NOT clear sand. All it will do is ball up with the sand and cause bigger issues such an enroliths
(sp
).
If you have a sand issue, you need to feed physillium as that bonds to the sand and clears it out of the system.
