What kind of wormer do you all use and when do you use it? I mostly use Equimax and Quest Plus in a rotation but I wasn't sure if I needed to mix up my active ingredients a bit. What do you all do and why?

Yes, active ingredients need to be rotated. That is what a rotation is all about.

I have heard that it is best to send a manure sample in to the vet and check for any worms or parasites before you worm to make sure it is necessary so worms don't get immune to all wormers, we still worm regularly but I thought I would just throw that out there. Has anyone else heard about this?

It is best to send manure samples in and only worm when needed as parasites can become immune to wormers. I would recommend rotating between the 3 classes of wormers.
We live in the Midwest with freezing winters so worm as listed below
Anthelcide in March
Quest plus in June
Panacur or Safe-guard in September
Equimax in December after ground is frozen
Some vets recommend worming every 2 months.
We do worm our colts every 2 months rotating between the 3 classes of wormers
I'm sure other people will have different ideas but this works for us with the climate we live in
