What are the good ones?

Having an issue with mine.  Haven't had to use it but they have ran my card on me 10 days before a due date AND it was only to be used the one time I called it in.
Next payment is due, but I received no invoice, no email back when I was asking when it was due and letting them know I had not got an invoice to pay.
Also, a friend used this company and waited for months to get paid on an approved surgery.  That would be a big deal to me depending on surgery cost.
  What are the good ones?
  What companies pay quickly?
  Thanks!

Is it normal for the insurance to only cover 60% of soft tissue injury?

Trying to do a little bit of homework before picking one.

I use Wilkins, but thankfully have never filed a claim. I have a friend that uses them also and just filed a claim on a horse that had a leg injury. They are paying for his entire treatmen and responded quickly to her.
