I have a horse that was diagnosed with EPM, we did a round of Toltrazuril for 30 days (double dose for first ten, regular for remainder)
The results with Toltrazuril were AMAZING. However, relapse occurred within the next few weeks.

Following this we treated with Marquis and I have not noticed near the same level of improvement. That finished a few weeks ago and the horse is still so-so.

I have visited with a few vets and am really not sure where to go next. Is there a real cure for this?


Major symptoms include - chronic head shaking, lacking coordination in hind end, lethargy, feels broncy (NEW)

If the results with the Toltrazuril were as good as you say I would hit the horse with another treatment round of the Toltrazuril. Some of these horses just require multiple treatments. I strongly recommend to clients to have their horses re-assessed after the initial treatment even if they have gotten significantly better- if there are still even subtle neurologic deficits I will generally recommend another round of treatment.

Another option to try would be Levamisole and Decoquinate- this treatment is relatively cheap compared to some others out there (about $300 for a 28 day treatment)... and we have been seeing some really great results with it.

My vet has a toltrazuril/dmso concoction that he uses. 4 tubes on day 1, 4 tubes on day 3 and you're done. I've seen several treated and no relapses that quickly. I treated the same horse 2 years apart and had great results both times.

Do you know the amounts? And once treated it lasted quite awhile?

I don't, he has a pharmacy compound them for him, I would not try to mix it at home. Equine Referral Clinic in Runnels, IA.

Both times my horses symptoms were minor but he bounced back beautifully and quickly. I treated him on April 1st of this year and the first weekend in May we clocked a personal best and pulled a check at a large jackpot.

My horse has had about six rounds of meds for EPM. Has done well after each. Vet ran a blood titter and determined the horse WAS NOT relapsing but getting reinfected. Opossums are particularly bad around here but barn cats are pretty suspect too. The last treatment that he had was Oroquin. He responded very well to that and has not had another treatment since. Not every horse is going to Desmond the same way to a medication.

My vet loves Oroquin for EPM treatment. It's a lot cheaper than Marquis.

It's been a few years but I treated my horse with Protazil for 30 days, saw improvement but not 100% so I treated him with Levamisole after having bloodwork sent to Pathogenes. 
The biggest thing that has helped my horse to not relapse in my opinion is diet.  He gets Renew Gold, alfalfa pellets or hay, and MVP Natural Vitamin E (5,000 IU). 
He is turned out 100% of the time unless the weather is BAD.
My horse was originally diagnosed with EPM and West Nile and I think the West Nile is what caused the most damage or caused the EPM to be more severe.  That part will never be known for sure.   
Keep trying and if your vet doesn't want to treat your horse the way you want him treated find a vet that will.

I have used diff treatments - by far the best is the recommendations for Pathogenes.com's medicine - Orogin (which is 2 meds combined decoquinate and levamisole) - its 10 days of treatment - and then if any symptoms still remaining they'll ship you 14 more days of just 1 of them (levamisole).. it works.. its cheap compared to the others.. and it works..oh yea.. it works.. :) had a horse relapse on the other drugs including marquis and the combo sulfa meds, pathogens has successfully treated 2 of mine and several of my friends horses...
