Horse is Cushings and IR but underweight

Subject: Horse is Cushings and IR but underweight

I haven't talked to the vet yet since it was my mom she called (it's my dads horse) so i don't know the numbers, but the 22 yo gelding (same one as my chronic diarrhea post) tested as being Cushings and having high insulin. Now I'm not sure what to feed him because he's also underweight. He already lives on a dry lot, and he was being fed beet pulp and alfalfa pellets with Timothy hay. I recently switched out the alfalfa pellets for Timothy pellets to see if that helped with the diarrhea. He also gets Horse Guard multivitamin.

On the body scale he's probably a 3, maybe a 4. You can see some of his ribs and the covering over his haunches is pretty poor, and his shoulder feels bony.

I'll ask the vet but what does everyone else feed their Cushings and/or IR horses?

  insulin resistant horses unfortunately should not be on a high fat diet. Mainly low startch. I have a Tb mare that is only 13 and is not over weight that has both Cushings and IR. She gets 4 lbs of alfalfa pellets and 4 lbs of triple crown low startch and close to free choice hay a day. She lost a little weight I think due to the prascend so we upped her feed to what I posted above. She has gained on it thankfully.

 80 % grass 20% alfalfa mix.

Mine has cushings and doesn't handle starch well and he does super well when I add rice bran to his feed. Right now he's just on a grass diet balancer and rice bran. It's high fat, but safe. I discussed with 2 vets and a nutritionist before coming up with his program. He eats alfalfa hay fine and I give him timothy pellets soaked just because he's on the grass diet balance and the grass in his paddock is eaten down now. I'm a huge fan of Empower Boost, it has even completely filled in the topline of a 30+ year old arab with no teeth at my barn, it was miraculous. They were about to put her down & she looks like she's 20 again honestly. They all get 2 cups of the Boost a day & holy crap the results have been amazing for all 8 that are getting it.
