I loop about 4 rubber bands together into a chain, then unhook my reins from the bit and put each end of my rubber band chain on the reins. I wind up with the rubber bands loosely connecting my reins under the horse's neck. Never have flipped them doing this and the rubber bands stretch so you can still use your reins. I'll see if I can find a picture.

I didn't realize so many people had this problem. What causes it to happen? Ive only had it happen once when my gelding tripped and pulled the reins out of my hand. I've seen people with thin rope that goes around their breast collar D ring and to the reins. Hard for me to explain without a pic. Never knew why though.

I use the smallest bungee you can find and connect it on each side of my reins. If I could post a pic, I'd show you.

My theory is if the reins reach your chest or waist .. It's gonna happen . Keep the reins to the pad or horn . Decent weight on the reins helps too.
