Never heard of this...
May I ask why a horse would need one other than an emergency and needing to unblock a restricted airway?

Scar tissue closing the airway causing the horse to not be able to breathe. I've spoke with one person who has a horse with one and she said it was the best thing she ever did. My vet suggest it, I just want another opionon

Yes, for horses with cicatrix which is what you are describing. It is inhumane to NOT do it in my opinion if they are at the point where they struggle to breathe because of the amount of scarring. It's not a big deal at all, isn't that gross to look at, and you can still ride/compete on them. Just don't let them swim or they can drown.

**they can't run on the track anymore though, its illegal to run trached horses on the track** but barrels, they still can.

We had a customer that had one done on her Western Pleasure horse. The horse was under for surgery and something went wrong, can't remember the details, but they had to do a trach. Very creepy to look at and you have to keep it clean and as far as competing, although they might be able to physically, you would have so many people creeping out, I can't see that working out. JMHO

There was a thread on here a few years ago about this with some good info. They do get pretty nasty and you need to keep them clean. I guess if it's the only way the horse can function then it's sort of a no brainer. But having had two trachs myself, I wouldn't wish them on my worst enemy. You really don't appreciate nose hairs until you've had a trach. It's like breathing fire. Without the air being filtered and moistened, it hurts the breath. I couldn't imagine running a horse like that. To me it would just be cruel to expect one to run. But I have a different perspective on it than most people.

We had a few horses get into a toxin that paralyzed their larynx's to the point that if the horses trotted they started to suffocate (very scary to see). We put trache's in. We had to clean them everyday but they reached a point it was either do a the ask surgery, or permanent trache. The downfall to a permanent trache is you are eliminating their upper airway system which is pretty much their way of filtering the air for any little dust particles that should not go directly to the lungs.

Had a grey broodmare with cancer/tumor in her throat and a foal by her side, vet gave her a trach but literally just a big hole in her throat done in the barn so not sure what a competiton horses would look like. Foal was young so we had to keep her alive and not let her suffer from not being able to breath so it was the best option and most humane. Made some weird noises out of it and it had to be cleaned with a rag sometimes but for the most part she was fine and happy. Growth kept getting bigger and cutting off more and more airway so we euthnized her after the colt was weaned about 2 months later

I have a friend that had tyo do it to one of her great horses. She ran at the NFR. Her little girl was running this horses for several years and was kicking butt doing so. Keep the horse in a heavy grass trap and no water that it could swim in. Clean the site which is not that big of a deal everyday.
If the horse needs it it's probably already past the time it should have been done. Don't delay.

One of mine had an emergency trach for cicatrix a year ago this December. It saddens me to think of all the trims she couldn't really catch a breath. She's 23 and I use her for trail riding only now, but the vets said she easily could go back into competition. She has done great. We cleaned it 2x a day in the beginning, but now just once a day.
