My good horse suffered a tendon injury at the BBR Finals. Some old scar tissue tore away from the superficial flexor tendon on his left front.
Please share any treatments y'all have had success with!! The more help I can get on this, the better.

Sounds like kind of the same issue I had happen to mine. I did PRP on mine last July 2014 and so far so good. Might look in to Novobrace.

 Stall rest!!! This is the most important therapy of all! Follow up with a strict exercise routine. I would give it atleast 11 months from injury to first run back. I did a sweat with poultice and also back on track. Started mine on smart tendon too!

Even if you do alternative treatments rest is still the key!

He's on stall rest. He's not happy about it, but he's managing. Lol. I also just started him on SmartRehab. He will be on that this month and then will probably be changed over to SmartTendon. I have been wrapping/poulticing/sweating. I'm just not sure how often I should be sweating vs poulticing.

 I just used poultice, wrap with a damp quilt, Saran Wrap, then a standing wrap. On for 12 / off for 12. If the heat is gone you can cease cold therapy and use heat therapy. The heat increases the circulation.  Somedays if you want to be lazy poultice alone would be fine. The way I see it just as long as you are doing something for it, it will help!

I would certainly go by what your vet advised but after the PRP treatment (did a series of 3 two weeks apart), he never went on stall reset. He was just in turn out for a month, after 30 days, he felt fine and vet stated to start an easy work schedule. He went to 2 really good expert lameness vets, both provided the same protocol. They advised that after the initial lameness/inflamation has subsided, stall rest does not allow for the tendon to regain some time of flexiblity, especially with scar tissue. Scar tissue needs to be allowed to become as flexible as it can during the healing process or your probability of reinjury is almost certain. All horses will heal differently but it's the advise I received from two great performance vets and it worked for my horse. I would certainly look into the Novobrace. I will be doing this if mine horse ever re-injures his.

Along with stall rest, I would also get him out daily and hand walk or put on a walker (if he will behave himself and just walk).  If he just stands the whole time he is healing, he will form more scar tissue.  The more you can get it to heal with less scar tissue the stronger and more elastic it will come back.  The scar tissue is rigid/stiff (non-elastic) and weak, so when put under pressure, it tends to tear.  I did this with my suspensory tears.

I've done stem cells and PRP. But if its just a minor bow (no tear, holes, etc) normally inject with adequan, predef, and betamethasone.
Give time off. Laser treatments. Cold water hosing am and pm.

I spent the money on runners relief and honestly don't think it helped a bit for the price. Maybe the soak works better than the poultice but I didn't feel like the poultice did jack squat. I like numotizine better wrapped 12 on and 12 off.

ETA: Before you do turn out or start hand walking make sure you talk with your vet. Ours wanted to re-ultra sound at certain intervals before we started hand walking, working under tack, etc... You do want elasticity but depending on the ultrasound your conditioning after treatment might be different.

What caused the old scar tissue?

Problem with scar tissue, is it is not elastic, if the scar tissue has tone, I would be asking why, as if the tendon is left alone more scar tissue will form causing less stable tissue which if not maintained properly will continue to retear.

I would be asking a qualified surgeon on my options, I would be asking if stem cells will benefit as scar tissue is already present.
