Go to walmart and buy omeprazole and ranitidine in the acid reducer section.

20 150mg ranitidine 2x a day
11 omeprazole 1x a day

Try this for 2 weeks and give him this time off. On NSAIDS ulcers will get worse so this could be why he wasn't better when using bute or banamine.

If this ulcer treatment does yield positive results, do you keep them on it? Is there a lower maintenance dosage, or can they only receive this for a two week period?

Not sure, but maybe you are anticipating him refusing to go in? When you tense up he can feel it and start acting up. Hope you can figure it out, it can be dangerous when they start rearing up. 

If you have checked everything else out and he is 100% then I agree that he could feel you anticipating like crankB410am said.

I had a gelding that would give me a hard time randomly like this. I noticed that when I would get tense so would he. I actually started walking him to the alley with one hand on the reins and one on his butt (leaning back as if I would be sitting on him relaxing chatting to a friend) and then when he got to the alley I would pick up two handed and go. Weird, I know, but it worked.

From everything you've already checked out....this really sounds like you're getting nervous and passing it to him.

I tend to ride hotter type horses and have been accused of being a 'nervous' person.... LOL! It's really difficult for me to 'just be still' when I'm ready to run. What works for me is to concentrate on my breathing a couple of runners ahead of me. I'll mentally go thru my run, always think of what you need to do, never what not to do. I'll take a deep breath and hold for 3 seconds and release. Very consciously relax your body, starting at your neck and down to your heels. I'll walk to the alley one handed and with a relaxed seat...when I'm ready to go, I'll pick up both reins.

My horse I'm currently running is hot and always on go when we're ready to run.....but if she doesn't want to go in, it's pretty much a guarantee she's sore somewhere. I'm fine with one being a fire breathing dragon in the alley, but really watch your body language if they get hesitant about wanting to work.

You don't want to use a maintenance dose of ulcer meds...They block absorption of other nutrients that they need.  Heal the ulcers, find the source whether it's pain or feed related, and you won't have to worry about ulcers anymore.  Maintenance is a bandaid for a larger problem...and will only cause other problems down the road.
