9 yr old horse had been running and coming in money in 1D or 2D. All of sudden started dodging 1st barrel. This went on for several years and I did take him to a sports vet and took care of a right shoulder issue she said he had. He still dodged. Horse had some ulcer symptoms so treated for ulcers for 33 days and gave 7 months off except for a practice run in 7th month to see if might still have issue and one of the runs appeared he had it in his mind but was corrected easily. Took to get scoped and he had some type II ulcer which they believe is Helicabactor Pylorie related and treated with antibiotics. Also, had lameness check even though he had NEVER SHOWN any lameness issue. Horse had barely been sat on for 8 months by now. Was given flexon test and trotted on hard surface to see if lame. I could see his back right pastern didn't travel exactly the same as his left but was so slight I was not sure. When blocked for the 2nd time which was just above the pastern the vet did an ultrasound and pointed out his sesamoid bone in the ultrasound and said he was pretty sure something not right but would have to do xrays to be sure. After xrays he showed where the very top side of the sesamoid bone looked like it had been chewed on and diagnosed him with Sesamoiditis and could operate but it may or may not work.
So had been told of this other vet that was well known surgeon and was going to have him operate, however; he said he could not see any abnormalities in his xrays nor could he operate on that anyway. I'm so lost. Anyone else had anything similar and what did you do. It must be something. Again, the horse has NEVER EVER been lame, just a 1st barrel issue which could have been from ulcer.
Any help would be appreciated.

I would take the horse and have another X-ray completed with a different machine.

Not all X-rays are created equal, and not all vets taking the X-rays use the appropriate amount of radiation.

I know this might seem pricy but I would have a body scan done. It was the best thing I did for my mare. Was chasing the issue around her body doing multiple treatements with no luck. The body scan pin pointed the issues and were able to be corrected.  It also let me know that all the previous diagnosis were wrong and that I was treating a part of her body that had no issue to begin with.
