OK finally had to break down and start injections, a couple weeks ago had left stifle and both outside knees injected, all was good, now both rear ankles down to the hoofs / cornet band are swollen. He's not lame or doesn't appear to bother him still runs and bucks across pasture. Could this be infection or a reaction to the injection?

Could be a joint flare, I would take the horse back to the vet, and get a culture just to rule out infection.

Just gonna throw this out there. But it may not be related to your injections at all. ....Could be scratches. ..Either way get to vet incase it is related to the injection

Did your vet tell you to stall rest him for 24 hours? Does he normally stock up when stalled? Any degree of heat in the injected areas?
