Long story short, I have a horse who had kissing spine surgery in March. Rode him lightly over the summer, but went back to the University that did the surgery 3 weeks ago and his back showed no signs of pain. He showed some signs of pain in his hocks and stifles (he's 17) so I had them injected. Under saddle now, he refuses to want to trot. I have a G-G saddle that I got and rode him in prior to surgery, but he was in pain, so I don't know if that hurt him prior to or not. I TRULY don't think this horse is in pain. I have come to know how he looks and moves. BUT, why is he refusing? He will start trotting sometimes on his own, but if I stop and ask him to, he will shake his head and stop? When I take the saddle off, there are no dry spots? Perhaps a little tight underneath the wither? I haven't yet tried a different saddle , I have tried a different rider and it was worse?

Does he move out okay on a long line unsaddled? What about with the saddle?

How does he move riding him bareback? Does he trot okay then?

he moves out perfectly, both with and without a saddle on a lounge line

When I first got on my horse that had KS surgery (extensively!), I noticed that he was fairly uncomfortable. I made sure to get him a really good pad (CSI), and get my saddle fit evaluated by a professional. It helped a lot, but what helped the most was just giving him enough time to completely recover. He got a year off and came back beautifully.
