Subject: Need help building an entry form

Can someone help me? I volunteered to do the entry form for a memorial barrel race we are having and I really have no idea where to start. I have never made an entry form before!

Location: Kansas		I have created several different forms in Excel. Shoot me your email and I can send you one of mine. The big thing is to leave room for people to write their name & their horse's name......I get tired of having to cram "Behold He Reigns" and "Royal Red Streaker" on to entry lines.
