Had 2 two year olds gelded last week. Have been working them lightly 2x a day, one has no residual swelling, the other has gotten quite large. He is also lame now on the left hind, stiff to walk and will hold it up while trotting or loping. Had him to the vet today for check up and antibiotics. Any one with similar experience? Could it just be the pressure from the swelling?

Are they stalled or in a large turn out?

They are in small corrals, room to move, but this particular one chooses not to. He does get controlled exercise 2x a day.

The best advice I can give is to make sure the signs are right. No bleeding. No swelling. Also I try to put mine out with a cranky mare. Those girls will make sure they keep moving.

He may need the incisions opened back up.  They need to move for them to drain and if they chose not too, sometimes they can seal up prematurely and not heal properly. Will probably need antibiotics also.

Sounds like his nut sack is full of pus. Is that possible?
I started getting sore just reading your description.

Does he look like he's draining? What did the vet say? My concern would be that the incision has a infection or sealed up so it cant drain. Did the vet need to reopened the incision? If its draining good, I would give a little bute so he would loosen up some and move around more so the swelling will go down with him moving around. I dont think hes lame he just hurts and need some pain control.

Vet here- I always strongly recommend forced exercise for all my post-op castrations starting the following day. 15 minutes of forced activity (lunging, long trotting etc). This promotes drainage and helps keep swelling at bay. Without exercise, especially if they stand around, some will get VERY swollen.

He has been exercised daily. The vet opened him up again yesterday, no pus or real signs of infection, gave him penicillin to be safe. He is on Equioxx, I now have him turned out during the day and forced exercise 2x a day. Still just as swollen and sore.
